Background: Treatment of psychotic disorders in pregnancy is often ethically and clinically challenging, especially when psychotic symptoms impair decision-making capacity. There are several competing ethical obligations to consider: the ethical obligation to maternal autonomy, the maternal and fetal beneficence-based obligations to treat peripartum psychosis, and the fetal beneficence-based obligation to minimize teratogenic exposure. Objective: This article outlines an ethical framework for clinical decision-making for the management of chronic psychosis in pregnancy, with an emphasis on special considerations in the previable and (...) periviable period. Case Presentation: A 31-year-old gravida 2, para 1 with intrauterine pregnancy at 12 weeks and 4 days gestation was brought to the emergency department by her husband seven months after delivering her first child, due to sudden onset of behavioral changes that included self-isolation, not eating, and not taking care of her child. Her past medical history included hypothyroidism and inflammatory bowel disease, but no prior psychiatric illness. After being admitted to the psychiatric hospital, she continued to have poor oral intake and weight loss despite initial inpatient treatment with antipsychotics, levothyroxine, and discontinuation of corticosteroids. Her pregnancy was also complicated by the diagnosis of multiple fetal anomalies at 20 weeks gestation, when the fetus was periviable. Conclusions: For previable or periviable pregnancies, the patient and/or surrogate should decide whether to pursue prenatal genetic screening and invasive diagnostic testing, as well as whether to continue or terminate the pregnancy. When the choice is made to continue the pregnancy, initiation of long-term psychiatric treatment (including medications with potential adverse fetal effects) should be based on shared decision-making between the physician and the patient and/or surrogate. Although some pharmacologic interventions may have potential adverse effects on the developing fetus, the use of psychotropic medications can be ethically justified, even if the patient herself does not have the capacity to consent and requires a surrogate, when the goal is to restore maternal autonomy and minimize the risks of maternal and fetal harm from untreated psychiatric illness. Sustained attention, a key cognitive skill that improves during childhood and adolescence, tends to be worse in some emotional and behavioural disorders. Sustained attention is typically studied in non-affective task contexts; here, we used a novel task to index performance in affective versus neutral contexts across adolescence (N = 465; ages 11–18). We asked whether: (i) performance would be worse in negative versus neutral task contexts; (ii) performance would improve with age; (iii) affective interference would be greater in younger adolescents; (...) (iv) adolescents at risk for depression and higher in anxiety would show overall worse performance; and (v) would show differential performance in negative contexts. Results indicated that participants performed more poorly in negative contexts and showed age-related performance improvements. Those at risk of depression performed more poorly than those at lower risk. However, there was no difference between groups as a result of affective context. For anxiety there was no difference in performance as a function of severity. However, those with higher anxiety showed less variance in their reaction times to negative stimuli than those with lower anxiety. One interpretation is that moderate levels of emotional arousal associated with anxiety make individuals less susceptible to the distracting effects of negative stimuli. (shrink)

At the intersection of meta-ethics and philosophy of science, Nicholas Sturgeon’s “Moral Explanation” ([1985] 1988), Richard Boyd’s “How to be a Moral Realist” (1988), and David Brink’s Moral Realism and the Foundations of Ethics (1989) inaugurated a sustained argument for the claim that moral kinds like right action and virtuous agent are scientifically investigable natural kinds. The corresponding position is called “non-reductive ethical naturalism,” or “NEN.” Ethical nonnaturalists, by contrast, argue that moral kinds are genuine and objective, but not (...) natural. This dissertation is largely a challenge to non-reductive ethical naturalism from the viewpoint of ethical nonnaturalism. An introduction, four fairly independent chapters, and a conclusion comprise this dissertation. The introduction (chapter one) situates and summarizes the arguments in the four chapters that immediately follow, and the concluding chapter (chapter six) describes future work related to the arguments in this dissertation. I now turn to chapters two through five. In chapter two, I shore up what I believe are weakness in Derek Parfit’s Triviality Objection to reductive ethical naturalism and then present a challenge to the NEN theorist, which, I argue, the NEN theorist must but ultimately cannot meet. The challenge is to sketch an empirical experiment whereby we can adjudicate between competing moral hypotheses. I explain in terms of experimental design and evaluation why the challenge cannot be met. Chapters three and four focus on the semantics and reference relations of kind terms. In chapter three, I defend the Causal Externalist (CE) theory of semantics and reference against an objection distilled from two articles by Åsa Marie Wikforss. The objection is that CE implies a paradox, escape from which forces the CE theorist upon the horns of a dilemma. I show how the CE theorist can avoid the dilemma by allowing that the intensions of some artificial-kind terms (e.g., ‘jade’) are not given by descriptions associated with the terms. One consequence of this move, however, is that it casts doubt upon the coherence of the claim that moral-kind terms (e.g., ‘morally required action’) are natural-kind terms. In chapter four, I defend Terence Horgan and Mark Timmons’s Moral-Twin Earth argument from three critical responses put forward in recent paper by David Merli. NEN theorists claim that CE can account for the semantics and reference of moral-kind terms. As per Horgan and Timmons (H&T), however, our responses to the Moral-Twin Earth thought experiment give us reason to doubt the NEN theorists’ claim. Against H&T’s argument, however, Merli presents three independent, critical responses, and in this chapter I defend H&T’s argument in a way that is friendly to the ethical nonnaturalist. One upshot is, I contend, that the ethical nonnaturalist can and indeed should co-opt H&T’s argument for his own use. In chapter five, I return to kinds themselves, specifically those in biology. I first point out that ethical nonnaturalism appeals to a metaphysical thesis about nonnatural value, which need not be limited to the moral domain. Then, I argue that biological naturalism alone cannot give a satisfying realist account of biological kinds like HEART and NEURON. Finally, I show that by conjoining the concept of Cummins functions as recently defended by Paul Davies, a thesis about nonnatural value, and Ned Block’s Disney Principle regarding structural properties, the nonnaturalist, unlike the naturalist, can offer a satisfying realist account of biological kinds. It is now generally agreed that in Aristotle's Poetics, ch. 13 means ‘mistake of fact’. The moralizing interpretation favoured by our Victorian forebears and their continental counterparts was one of the many misunderstandings fostered by their moralistic society, and in our own enlightened erais revealed as an aberration. In challenging this orthodoxy I am not moved by any particular enthusiasm for Victoriana, nor do I want to revive the view that means simply ‘moral flaw’ or ‘morally wrong action’. I shall (...) try to show that the word has a range of applications, from ‘ignorance of fact’ at one end to ‘moral defect’, ‘moral error’, at the other, and that the modern orthodoxy, though not as clearly wrong as the moralizing interpretation it displaced, restricts Aristotle's meaning in a way he did not intend, and does lessthan justice to his analysis of classical drama. The strange series of hypodochmiacs here and atO.T.1207 ff., with brevis in longo without pause atAj.421 andO.T.1208, seems metrically self-contained, despite their syntactical interdependence (esp.Aj.421–2οὐκέτ' ἄνδρα μὴ | τόνδ' ἴδητ', so that the word-overlap ofοἷονinto iambics in Pearson's text is unlikely.ἑξερῶ μέγαshould therefore be writtenplena scriptura. Thenοἷον οὔτιν' ἁ Τροί|α στρατοῦ…is possible, but the ithyphallic with word-overlap, sometimes found in the syncopated iambics of Aeschylus, is foreign to Sophocles. Divideἐξερῶ μέγα, | οἷον οὔτινα | Τροία…Thenϕίλοι τοῖσδ' ὁμοῦ = (...) οἷον οὔτινα, i.e.δ= hyp., which is not certainly found, and the antistrophe has two syllables extra. (shrink)

In her masterly article on this passge, Dr. Christiane Sourvinou-Inwood goes most of the way towards solving two serious problems: the text of Lys. 645, where the vulgate makes the ‘bears’ more than ten years old, contrary to all other evidence; and the meaning of of A. Ag. 239 . She argues cogently that in Aeschylus means ‘shedding’ the saffron robe, as most editors including Fraenkel have thought, and not ‘letting her robes fall to the ground’ as Lloyd-Jones, followed by (...) It has long been accepted as a principle by editors and writers on Greek metre that brevis in longo and hiatus in tragic lyrics often coincide with some kind of sense-pause. The object of this inquiry is to determine the incidence of pause in such places, and show that it is significantly high; to show that there is a comparable incidence in the corresponding places in strophic systems; to show that period-ends determined by criteria other than brevis and hiatus are (...) attended by similar conditions. It might seem that if all this were true it would have been recognized long ago, particularly as the connection between sense and metrical structure, and symmetry of sense in strophe and antistrophe, has often been pointed out. Orestes has revealed himself to Electra and sworn with her to avenge Agamemnon. He outlines his plan and leaves the stage with a prayer to his father, after warning the chorus against indiscretion . They begin: Earth nurtures many dread hurts and fears; the sea's embrace is full of monsters hostile to man; lights in mid-air between earth and heaven also harm winged things and things that tread the earth; and one might also tell of the stormy wrath of tempests. One day he went on a journey to a strange village far away in the plains, and there he saw a group of men sitting round another story-teller. In CQ 55 , 22–5, E. Harrison noticed that hiatus between verses in the trimeters of dialogue was much less frequent in tragedy when the sense ran on from one verse to the next, than when there was a pause in sense at verse-end. He observed that Aeschylus' Prometheus differed from the other plays of Aeschylus in this respect, the proportion of run-over hiatus to end-stopped hiatus being much higher, and more like that of comedy; that Sophocles had remarkably few (...) verses with run-over hiatus in the Trachiniae and Antigone , much less than Aeschylus in proportion to the number of non-stop trimeters in the play, though Oedipus Tyrannus had much the largest number in Sophocles , and no continuous chronological development was discernible; that in Euripides there was a general progression from relative strictness to relative freedom in the run-over hiatus allowed, though individual plays did not conform closely to this pattern; that in general comedy was freer than tragedy. This paper argues that, for Richard Titmuss, the rationale of the gift relationship (TGR) as a national blood policy is to reconcile liberty with social justice in the provision of an essential health resource. Underpinned by a needs-based distributive principle, TGR provides a social space for a plurality of values in which to engage with and motivate people to voluntarily give blood and other body materials as a common good. This understanding of TGR as a value pluralistic framework and its (...) implications will be used to discuss the issue of using economic mechanisms to increase the supply of body materials or goods, including organs for transplantation. It is argued that, while TGR excludes a policy in which body goods are treated as private commodities and distributed primarily on the basis of achieving market efficiency, it is not in principle opposed to the use of material rewards, including financial ones, to motivate people to donate. Some years ago, Sir Kenneth Dover suggested a new interpretation of καρξαι. Prima facie, the chorus ask the sun to proclaim where Heracles is, and this sense is supported by such passages as Il. 3.277 Ήλιóς θ', ς πντ' ορς, Od. 9.109 Ήελου, ς πντ' ορ , Od. 8.270–1 αρ δ ο γγελος λθεν | Ήλιος, and especially h. Cer. 69ff., where ‘Demeter visits the Sun and implores him, “you who look down on all earth and sea…tell me truly of (...) my dear child, if you have seen her anywhere, who has gone off with her…”.’ This is the way καρξαι in Trach. 97 has always been taken. Dover points out, however, that κηρττειν also has a special, technical sense: to make proclamation inquiring about a missing person's whereabouts, as the town-crier used to do a century ago England and elsewhere, and the media do now. The model is not that of h. Cer. 69ff., but rather S. Aj. 845ff.: ‘Sun, when you see my native land, draw near and tell my aged father…of my fate.’ The examples he cites are enough to demonstrate the ‘interrogative’ use of κηρττω, though his first example, Ar. Ach. 748 γν δ καρυξ Δικαιóπολιν π, will not do: if sound, it means not ‘I will find out by κρυξ where Dicaeopolis is’ , but ‘I will summon Dicaeopolis to where ’. The normal ‘interrogative’ use is to enquire by herald the whereabouts of a Crminal or a runaway slave. (shrink)

It would be generally accepted that G. E. Moore’s celebrated “Refutation of Idealism,” set forth at the turn of the century, constitutes the classic statement of modern realism. The seeming strengths of this position have been elaborated more recently by a notable realist proponent, Don Locke, who, following Moore, takes for granted what is, in effect, the basic assumption of the “Refutation”—the assumption, namely, that each and every variant of the idealist standpoint is embraced under the central Berkeleian contention that (...) “esse is percipi.” It will be my object to argue that the plausibility of Locke’s realism stems from a basic confusion of terms, and that, in its implications, the position becomes vacuous when set in opposition to idealism.
